For example: after moving into the new office, the CEO repeatedly reprimanded us for not storing our belongings in lockers (I didn't realize this was 2nd grade??) as he wanted the office to appear clean and sleek. Meanwhile, all of the conference rooms had major acoustics issues that were so problematic, our clients couldn't hear us during conference calls. We often had to resort to using a cell phone on speaker and passing it around while presenting work. This went on for months and made us look like total amateurs.
