Summit Community Care Clinic was established in 1993 as a one night a week walk-in clinic staffed entirely by volunteers and managed by Summit County Public Health. Over time we added extra evening hours and eventually day-time hours. In 2006 we became an independent non-profit and separated from Summit County Public Health into our current space. Over time our services have grown to include medical, reproductive health, oral health, counseling, physical therapy, and group preventative health services. The Care Clinic has been operating as a Federally Qualified Health Center facility since 2015. Summit Community Care Clinic located in Frisco Colorado. In 2018, the Care Clinic served over 8,000 patients with 26,000 primary, dental and behavioral health visits. Patients are primarily from Summit County but there are a growing number of patients seeking services from Eagle, Lake, Grand Park and Clear Creek Counties. Mission: The mission of Summit Community Care Clinic (“Care Clinic”) is to provide exceptional, integrated, patient-centered health care services designed to meet the needs of people who experience barriers to accessing care, regardless of their ability to pay. Our goal is to improve the health and well-being of our patients by preventing illness and treating disease in the most comprehensive and compassionate way possible.
