Vital Decisions

Serious illness can be overwhelming with many difficult decisions to be made. Given the dynamic of our healthcare system, often these decisions are made without sufficient consideration of the priorities and values of the patient. Unfortunately most patients and their families don?t recognize the need, have the knowledge, or possess the confidence to bring about their preferences. Vital Decisions? goal is to help seriously ill patients participate in, and exert more control over, decisions about their care. Mission:?Our Living Well Program works with individuals and their families so that they are able to proactively participate in their care by identifying, incorporating and communicating their priorities into current and future care decisions. Our mission: Vital Decisions is the force, the catalyst in this country which is fundamentally changing the care delivered during advanced illness.
