The Center for Elders? Independence helps at-risk seniors live at home, instead of in nursing homes. We support our participants? medical, social, emotional and nutritional needs. Our care teams include 11 professional disciplines. Every week, they meet in one room to review our participants? care plans and changes in medical conditions, social engagement and emotional well-being. Although there are never any co-pays for services or medications, CEI-PACE will be the participant?s sole service provider & CEI guarantees access to services, but not to a specific provider. This model of care is the Program for All-Inclusive Care for the Elderly (PACE). Our sister organization, On-Lok in San Francisco, created this care model 45 years ago. Today, 35 PACE centers in California take care of over 6,500 participants. Mission:?The Center for Elders? Independence provides high quality, affordable, integrated health care services to the elderly, which promote autonomy, quality of life and the ability of individuals to live in their communities.
